New Patient Information
We welcome all new patients and thank you for trusting us with your dental health. Please ask your former dentist to send a copy of your records to our office.
How to prepare for your appointment:
Please be sure to bring your dental insurance card and arrive early to fill out your initial paperwork if you have not already done so. (These forms can be downloaded above)
If you are on any medications, please bring a list of them with you to the office
If you need antibiotics prior to dental work, please take them as prescribed. If you are not sure if you need antibiotics, please contact your doctor prior to your visit

Digital x-rays have many advantages compared to film x-rays. First, they reduce the amount of radiation needed. The improved diagnostic capability of digital x-rays and the ability to view the x-rays on a computer screen allow the patient to better understand and follow treatment. Digital x-rays are both eco-friendly and a time saver! They are instant, with no need to develop film, saving the traditional supplies used and chemical waste.

Using Electronic Medical Records is a wonderful addition to our practice. We are able to quickly and accurately access patient information, ensuring patient confidentiality without excessive paper waste. This digital format allows for quick access to your information for both in-office and insurance purposes.

This new technology allows both the patient and the dentist to see detailed images of your teeth and mouth during your appointment. With an intraoral camera, you're able to better understand what is happening in your mouth and review the status of your oral health.

These television monitors are found next to every patient chair. Patients can watch a movie or TV show. Patients can also view their dental x-rays when speaking to the doctor about the findings for a better understanding of their oral health.

Financial Information
We provide the finest dental care at a reasonable cost to our patients; therefore, payment is due at the time services are rendered. Forms of payment accepted by our office are check, cash and major credit card (American Express, Visa, MasterCard and Discover).
For patients who require major dental treatment, a monthly payment plan can be designed with an appropriate payment schedule. We are happy to make arrangements for a monthly payment plan but the plan must be implemented prior to commencing treatment.
CareCredit
We're pleased to offer our patients with the CareCredit card, North Americas leading patient payment program. CareCredit is the credit card exclusively for healthcare services. You'll get low monthly payments for healthcare procedures not commonly covered by insurance, including dental procedures. Plus, you can use your card over and over for follow up appointments.
